Participating Insurance Companies

Burkhart offers complementary billing to patients with health insurance. Patients may be responsible for a co-insurance, deductible amount, or full payment depending on individual plan and coverage. Out-of-network benefits and deductibles will apply for non-participating insurance companies. Personal injury and auto insurance may be billed on a case-by-case basis. We do not accept assignment on insurances we do not participate with, and we do not participate with Medicaid. Direct and prompt payment is expected for non-billable services rendered.
